Prescription refills for chronic disease management, mental health medications, hormone replacement therapies. 

 *For controlled substances you must book an initial in-home, face-to-face new patient evaluation. This is to satisfy the federal requirement for prescribing of controlled substances through telemedicine. Please book the appropriate appointment under "Mobile Medication Services" at a rate of $125. Medications shall be approved based on proof of previous diagnosis by a mental health provider and/or evaluation of comprehensive history obtained during your visit. Medications to be prescribed are at the judgement of the provider. If you are deemed to be at high risk of abuse of the medication, then the provider has the right to refuse prescription and the money for your appointment will be refunded. Patient will need an appointment every 3 months for the management of controlled substances and can be conducted via telemedicine for all subsequent visits.

• Patient will need to be able to email or fax FMLA or short-term disability paperwork to provider prior to appointment. 
• Paperwork will be completed and sent to appropriate entity within 24 hours of appointment.
